Our Health and Wellbeing Coaches are here to help you get more active and lead a healthier lifestyle. They encourage you to take control of your health and support you in making long term lifestyle changes. They are available through each of our practice reception teams or you can contact them online using the self-referral form.

Somerset Activity and Sports Partnership provide health and wellbeing coaches to support people with long term conditions to access physical activity.

If you are living with a long term health condition, at risk of diabetes or just ready to take control of your health, the Health and Wellbeing Coaches can give you the support you need to make healthy changes to your lifestyle.

The coaching service is available to anyone registered with these GP practices:

  • Taunton Road Medical Centre
  • Redgate Medical Center
  • Somerset Bridge Medical Centre
  • North Petherton Surgery
  • Polden Medical Practice
  • Cannington Health Center

Your coach will work with you to find ways to build more health-giving movement and social activities into your lifestyle. You can access to a wide range of fun and fulfilling activities that are free or affordable, giving you more confidence to build your healthy future.

Complete Community Care Project

Bridgwater Bay Primary Care Network (PCN) and Somerset Activity and Sports Partnership have partnered to provide advice and activities to new and young families. Lots of the new activities are fun and free, and will help to improve the health and wellbing of mums and their children.

Group programmes

If you would like support with the social aspect of meeting others in similar situations, you can attend one of the group programmes. Examples include 12 week Weight Management, Women’s Wellness and Healthy Living with Pain. Check the SASP activity finder for sessions near you.
