Our Village Agents are here to help you find community-based solutions. Thet can put you in touch with local groups and community services. Village Agents are available through our practices or you can contact them online using the call back service.

Village and community agents

The Community Council for Somerset (CCG) provides a team of local experts who help people in the community. They support the community by bringing services and people together. These are the village and community agents roles:

  • Village agent
  • Community agent - referral from adult social care only
  • Cancer support agent - help you access practical, financial and emotional support
  • Transport village agent - working on short and long term transport plans for Somerset
  • Homefirst agents - supporting transition from hospital to home

Village and community agents provide confidential, practical community based solutions across Somerset. They provide help and support to:

  • carers
  • people living on their own
  • young people
  • families
  • older people
  • community groups

Village and community agents can be contacted directly, by referral through your doctor's surgery or through the online agent call back form.

Talking cafes

Talking cafes provide information, advice and support to people and communities across a number of Somerset locations. If you can't get to a talking cafe, they offer online information on Tuesdays, Wednesdays and Thursdays at 1pm through talking cafes FaceBook.

Help with food

If you would like help with buying, preparing or cooking food, village and community agents can help. They are part of the Somerset Food Resilience Taskforce.
